Dorset CCG are calling for anyone who has had experience of end of life care in Dorset over the last three years to come forward and tell them about their experiences.
With the number of people dying in hospital reducing and increasing in other settings as well as at home it is important that patients and carers across Dorset have consistent access to quality care with a focus on what really matters to them.
The CCG is working closely with other organisations across Dorset including BCP Council, Domiciliary Care providers, Dorset Healthcare, Forest Holme Hospice, Lewis-Manning Hospice Care, Macmillan Unit, Marie Curie, University Hospitals Dorset NHS Foundation Trust and Weldmar Hospice care. Together, we are looking to develop a potential palliative care in the community service.
Your opinion matters. It is key that they understand what matters to patients, families, carers and the staff who deliver of end of life care, as part of the planning process for this new service.
There are a number of ways that you can share your story with them. They want you to feel as comfortable as possible when talking about this sensitive subject.
